Application Information

Note: Payment must be made prior to your desired class date to secure your spot in the class.

There are three ways to apply for one of our classes. If you are paying by check, please follow this link to our application. Simply print the application, fill it out, and mail it to us with your check. Instructions are on the application.

If you are using American Express, Visa, Master Card or Discover to pay for the class, you can follow the link above, print out and mail or fax the application to us (instructions are on the application) or you can call us with your information.
Please call us at 816-478-3677 to sign up.
If you call us to sign up, we will fill out your application for you.

If you would like to sign up on-line, simply copy the above information, paste it into an email and send it to us at: rollingwheels@sbcglobal.net. You must call us with payment prior to the end of the next business day to ensure your spot!

Students must complete the course with the class with which they began the training. If the instructor determines the student is not eligible to complete the class or if the student voluntarily decides to leave the class, refunds and/or rescheduling are not available. If the student is late for any class session, the student will not be able to participate and will forfeit his or her enrollment fee.
